Follow Work instructions BBY4 WI-00003

Fill out BBY4FCD-00055 and attach to the job images as Internal when fully filled out

It is a legal requirement that adverse results on a water sample taken from a registered drinking water system are to be reported to the client, the appropriate Environmental Health Officer/Drinking Water Officer, and the Medical Health Officer contact for the appropriate region.  For non-registered water systems only the client needs to be contacted, as a courtesy.  Record the communication for registered systems on the Positive Coliforms in Drinking Water – Immediate Reporting form (BBY4 FCD-00055).  Record the date and time of the call, the name of the person contacted, and what information was provided.  Once the form is completed it is to be scanned and attached to the job within LIMS.

Order of Contact:

  1. Contact the client.  Client contact can be found under the “report to” section of the CoC.
  2. Contact the Environmental Health Officer/Drinking Water Officer
  3. Contact the Medical Health Officer

Special instructions for reporting to First Nations Health Authority listed in the Work Instructions (last 2 pages)

If your client is submitting Drinking Water samples consistently, suggest adding the after hours contact information in the Client remarks in CLNTc module, and under the client notes in BVS, as well as MHO and DWO info.

For Courtenay submissions (testcode CTEMPNI-W), check the job remarks first, before contacting the client, as the lab staff in Courtenay may have already contacted the client on our behalf. Courtenay uses CTY FCD-00013/1 for communication log.
