Sign all pages of the chain of custody.

When submitting samples via a paper chain of custody (COC), or providing an Electronic Transfer Record (eTR) associated with an eCOC submission, it is important that all pages are signed and dated by the individual relinquishing custody of the samples being submitted.


The name, date and signature on the chain of custody provides a sample transfer history record of the samples associated with that chain of custody.
